Check Insurance Benefits

Please note that not all dietitians participate in Medicare or Medicaid programs.
*Coverage for nutrition services is dependent on policy and plan through your insurance company. Please contact the customer service number on the back of your card to understand your benefits. While most plans are not subject to a deductible or other out of pocket costs, remember to note if you have a deductible and/or a co-pay, as well as the level of coverage (100%, 90% etc.). If you want to use insurance you MUST provide your insurance information at least 5 days prior to the time of your visit.
Complete Online Paperwork
You will receive several forms via email for you to complete prior to your appointment. Please make sure to complete everything no later than 2 business days prior to your 1st appointment.

What to expect
The 1st visit is approximately a 1-hour session. This visit is structured and designed in order to get to know each other and start your health journey.
The follow-up appointment(s) are available to answer any questions that may have come up. By meeting throughout the program, we will continue to explore and review your previous health goals. During this appointment, you will also set your new weekly health goals. These sessions range from 15 minutes to 1 hour depending on the service.
